How local authorities can get involved

This online toolkit provides comprehensive resources for all local authorities to access the model, share learning, assess local readiness, and explore next steps with partners.

While the programme has focused on older people, the principles have far-reaching applications including children's social care, housing, homelessness prevention, and community safety.

This programme, alongside many other important initiatives underway in the sector, represents an important step towards preventative public services, systems, and state - establishing common approaches to prevention, evidence gathering, and evaluation, so laying the groundwork for transformative change.

The multi-council initiative will be led by Partners in Care and Health, working closely with Community of Practice participants and the Department for Health and Social Care.
